This module is designed to support primary care to  develop strategies to access and use health information about their patients effectively, with the aim of implementing an improvement in health equity. 
By the end of this course, you will:

  • Understand a range of data tools available to primary care to segment or stratify your population by healthcare access and outcomes
  • Be able to manipulate data outputs from data tools linked to your population
  • Identify a potential population experiencing health inequity where an intervention could be delivered locally.
  • Implement links between service providers in your primary care landscape to tackle the identified health inequality locally.
